远程公证（Remote Online notarization）行为的电子公证证书必须：
（1） 明确该公证行为为远程公证行为；
（2） 包括一个空白处，远程公证人可在其中表明远程公证行为中的委托人是否根据本章第4（a）（1）或4（a）（2）节在远程公证人面前出庭；和
（3） 请指定：
（A） 印第安纳州偏远公证人所在的城市和县
远程公证人执行远程公证行为时；和
（B） 委托人签署文件时，委托人实际所在的城市、县、州或省以及国家。
（b） 填写以下任一表格符合本节的要求：“印第安纳州
